Explaining the Your Income tool

We have been working with primary care academics, experts and GP leaders to develop the Your Income tool.

This is intended to show you how your NHS income compares with that of practices within your CCG or your previous year’s performance.

The tool uses raw official data from NHS Digital – ‘NHS Payments to General Practice’ – which captures data paid directly to GP practices through ‘Open Exeter’ and the ‘Single Finance Environment system’ to cover local enhanced services payments.

The data are all publicly available and updated annually.

Comparisons

As part of the Your Income tool, you can see how your practice’s income compares with your previous year’s income or the CCG average income.

All the figures you see – except on the green panel, which shows the total funding for your practice (see below) – follow this equation:

X=Funding per patient in your practice
Y=Funding per patient in the CCG average/your previous year’s funding
Z=The number of patients in your practice

This calculation adjusts for the effect of patient list size to allow meaningful comparisons of the income figures. By using your own patient list size, and creating an average comparator practice the same size as your own, it gives you an at-a-glance snapshot on whether your practice is earning what it can.

In the comparison graphs, we have only included the funding pots that you are able to change, and those that continue to exist. For example, in the data on 2020/21 funding, you will see on the left of this page that you might have received income for extended hours, or unplanned admissions enhanced services. However, these no longer exist.

Your Income panel

The green panel at the top of the page presents income data taken directly from NHS Digital.

It shows exactly how much money you received from the NHS in the past financial year, in total and for each of the categories of funding pots (see below for details).

In addition, the panel highlights your practice’s ‘Total Real Income’ and ‘Average payment per registered patient’ after subtracting premises, dispensing fees and locum reimbursements, as these payments can distort the figures and thereby any comparisons.

For example, premises arrangements can vary greatly among practices. Thus reimbursement payments might include rent reimbursements, service fees, or even fees for renting out part of the premises. As a result, any comparison between practices may be biased by these payments.

Dispensing fees reimbursements have been removed from the comparison calculations, as these only apply to dispensing practices and would also skew the figures significantly.

Locum reimbursements have also been removed because these are a direct ‘in and out’ payment and reflect a straight reimbursement when locums are necessary.

Other ‘in and out’ payments, and funding that is more applicable to certain types of practices, have been included as these have limited potential to distort overall figures, and offer some value for comparison between practices.

Excluding practices

We have excluded APMS practices from the tool, as their payments vary significantly from those of GMS and PMS practices.

We also excluded from the ‘peer’ practices comparison any practice that was deemed to have ‘atypical characteristics’ by NHS Digital. This includes practices that merged throughout the financial year, saw a big increase or decrease in patient numbers or – for whatever reason – only had a partial year’s worth of data in the NHS Digital ‘Payments to General Practice’ data.
